Prevention

Proactive measures to minimize PDPH risk

⭐ Grade A-B Evidence

Needle Selection

✓ Recommended Practices

  • Use noncutting (atraumatic) needles for all lumbar punctures
  • If using cutting needles: Insert bevel parallel to spine long axis
  • Use smallest gauge feasible

⚠️ Not Recommended

  • Routine prophylactic bed rest Grade D
  • Routine prophylactic EBP Grade I
  • Routine intrathecal/epidural substance injection Grade I
2

Conservative Management

First 24-48 hours of treatment

⭐ Grade B Recommendations

1. Multimodal Analgesia (unless contraindicated)

Acetaminophen: [650-1000 mg PO/IV] q6h (max 4g/day)
NSAIDs: [Ibuprofen 400-600 mg q6h] or [Ketorolac 15-30 mg IV q6h]

2. Caffeine Therapy (within first 24 hours)

Dose: [300 mg PO] or [IV: 500 mg caffeine sodium benzoate]
Maximum: [900 mg/day] (200-300 mg if breastfeeding)

⚠️ Avoid multiple caffeine sources to prevent toxicity

📋 Grade C Recommendations

3. Hydration

  • Encourage oral fluids
  • IV fluids when oral intake inadequate

4. Bed Rest

  • May use for symptomatic relief only (not curative)
  • Not recommended routinely

5. Short-term Opioids

  • Only if multimodal analgesia ineffective
  • Oxycodone: 5-10 mg q4-6h PRN or equivalent

❌ Grade D - Not Recommended

  • Abdominal binders
  • Aromatherapy
  • Long-term opioids
  • Spinal/epidural morphine
3

Interventional Management

Advanced therapeutic options

🎯 Grade C Evidence

Greater Occipital Nerve (GON) Block

  • Indication: PDPH after spinal anesthesia with 22G needle
  • Technique: 3-5 mL of local anesthetic Bupivacaine 0.5% or Ropivacaine 0.75% + steroid at occipital nerve bilaterally
  • Limitation: May require repeat; severe headaches may still need EBP

❌ Not Routinely Recommended

  • Sphenopalatine ganglion blocks
  • Acupuncture
  • Epidural dextran/gelatin/hydroxyethyl starch
  • Fibrin glue (reserve for EBP failure only)
4

Epidural Blood Patch (EBP)

Definitive treatment option

⭐ Grade B-C Evidence

Indications

  • PDPH refractory to conservative therapy
  • Headache impairing activities of daily living
  • Severe neurological symptoms (hearing loss, cranial neuropathies)
